How to Fill the Aadhaar Card Enrolment Form
Step 1: The Aadhaar enrolment form can be downloaded directly from the UIDAI website or obtained from the nearest Aadhaar center. This two-page form comprises nine fields on the first page to complete and instructions on the second page.
Step 2: Pay attention to the written guidelines on the form and fill in all the essential details carefully to avoid mistakes.

Step 3: Prepare and attach competent, valid, and approved supporting documents that hold proof of your identity, current address, and date of birth. In case you employ an introducer or the Head of Family (HoF), submit proof of your relationship with the HoF.
Step 4: After completing the form and affixing the supporting documents, visit the nearest Aadhaar center to submit your application.
Step 5: The expert staff will capture your biometric data such as your iris and fingerprints and photograph you. This data, along with the information entered in the form, will get saved in the database.

Neatness Counts: Keep the form as neat as possible and avoid ink blots as illegible information may affect details on your Aadhaar card.
Fields to Fill
Full Name: Enter your full name, including first, middle, and last name, and ensure that it matches the name stated on your proof of identity.Gender: Indicate your gender, whether male, female or transgender.
Age/Date of Birth: Input your birth date in DD/MM/YYYY format. If unsure of the exact date, provide your approximate age. Tick the box that says 'Declared' if there is no proof of birth date, otherwise tick 'Verified' if you have supporting documents.
Address: Enter your postal address as written in your address proof. The same address will be used for delivery of the card. Don't forget to provide your email and mobile number in this field.
Details of Family Members: Provide the name and Aadhaar number of your father, mother, husband, or wife if applicable. For children below 5 years old, details of parents/guardian are mandatory.
Consent: Grant UIDAI permission to use your biometrics for authentication and sharing of information with public/welfare agencies by ticking the box provided.
Documents Attached: List the identity and address proofs you attached with the form. If you provided details of the introducer/HoF, tick the box accordingly.
Signature: Affix your signature or thumb impression in the designated area to signify your consent. This form can also be used for updating existing Aadhaar card details by providing the 12-digit Aadhaar number and the specific details to be changed.
